How we personalise care

When building a care and support plan for your child or young person, we take time to understand everything that’s important to them and you as a family.

We put what we learn into one central booklet called All About Me folders. This is a vital resource for anyone who helps to support your child and your family. It includes vital information, such as:

  • Your child’s unique complex care needs
  • How they like to be supported
  • Your family dynamics and relationships
  • Their interests and hobbies
  • The goals they want to achieve
  • Anything else that’s important

This folder is designed to grow with your child or young person. It is constantly reviewed and updated to ensure it always reflects the individual and their family. Watch our video to learn how these booklets help us provide personalised care!

Working with you and your family

We actively encourage individuals and their families to contribute to the All About Me folder so it’s full of information that matters most to you. After all, you know your child or young person better than anyone else.

We listen to parents and work with you to ensure your child or young person receives reliable, compassionate and truly personalised care.
