Nourish helps nurture person-centred support!

In each of our homes across the UK, we’re dedicated to providing high quality person-centred care and support. As part of this devotion, we’ve reinforced our incredible support teams with innovative technology. Nourish is a sector-leading software that empowers our colleagues to efficiently co-ordinate and manage care for the people we support.

By updating essential information in real time, our care is becoming even more person-centred. Our compassionate, specialist approach is evolving along with our technology and is having a big impact on the lives of the people we support. With accurate, accessible information on what they need each day, they’re empowered to live the fulfilling lives they deserve with the confidence that all their needs are supported.

Nourish real lives in real time

Nourish enables us to efficiently plan everyday care for the people we support. By recording important information in real-time, it ensures consistency and clarity in what individuals need each day. Whether it’s emotional support or personal care, attending an appointment or embarking on an adventure, Nourish helps our teams to consistently ensure each person’s support is truly tailored to them.

At one of our supported living homes in Staffordshire, our colleagues have embraced Nourish, ensuring they’re using it as effectively as possible and delivering even more meaningful care to the people we support. Led by Manager Kelis Underwood, the team have increased their confidence by attending additional training sessions and seeking bespoke advice from our dedicated Nourish implementation team.

Kelis wanted to ensure they were using Nourish so their records clearly reflected their person-centred support. Through their training, they gained a better understanding of how to make daily notes more meaningful and person-centred. A key focus was capturing individual choices, involvement in decision making, experiences, and achievements throughout the day. Through this approach, they could acknowledge both significant milestones and smaller successes. Each of which is equally important to the people we support.

Evidencing person-centred support empowers our teams to see the positive outcomes the people we support are achieving. Through daily notes, they can trace the quiet successes that create independence. Kelis told us:

“With Nourish, we see a richer picture of each person’s journey, highlighting what matters to them, how they are progressing towards their goals, and the achievements they accomplish every day. This supports a more person-centred culture and ensures every housemate’s voice is reflected in the care and support they receive.”

Person-centred for our people  

Within Nourish, our teams can record how each person we support likes to be interacted with, including their communication preferences. So, the prompts and questions our colleagues see are always relevant to the person they’re supporting. Kelis told us: “By tailoring interactions and improving the quality of recording, we’re building more accurate pictures of each person’s experiences and achievements, ensuring their support remains focused on what matters most to them.”

Nourish has also encouraged our support colleagues to reconsider what progress is by focusing on small, achievable steps that make a real difference. Kelis and her team supported one individual to fulfil their goal of attending an accessible disco that suited their needs and preferences.

Previously, finding the right disco was challenging. There were few suitable events nearby, and many of them included other activities, like games and bingo. What the person we support really wanted was to dance and enjoy music in an environment where they felt comfortable. Crowded spaces are overwhelming, which added another important consideration.

With support from the team, they attended several different discos to explore what would be suitable. After each visit, detailed notes were recorded on Nourish, capturing their feedback, reactions, and level of enjoyment. This information enabled the team to understand what was important to the individual:

“Using the reporting functions within Nourish, we reviewed this information over time, informing our approach to finding an event that met their specific preferences. They’re now attending regularly and enjoying the experience.”

Achieving this goal represented a significant outcome for the person we support. It demonstrated the value of listening, documenting, reviewing, and acting on her feedback. This collaborative and person-centred approach, coupled with effective recording on Nourish, supported them to achieve something deeply meaningful.

Nourishing connections

We understand family and friends are important to someone’s wellbeing and happiness. Introducing Nourish has also transformed the way they are involved in the people we support’s care. For Kelis and our team, one of the biggest benefits they’ve experienced is enhanced communication with families and how they coordinate activities in the community.

“Many housemates enjoy regularly accessing the local community and being out with their Support Workers. This promotes independence and community inclusion, but it sometimes creates challenges when family members visit and their relative is out. Nourish has changed this. We’re updating records in real time and can see where people are. So, when family members contact us or arrive for a visit, we can provide accurate information about where their loved one is and when they’re likely to be home.”

For the people we support, this streamlined communication has created additional opportunities for meaningful moments. Empowered with real-time knowledge, families have been empowered to meet their loved ones in the community. Potentially missed visits have transformed into positive shared experiences!

Using Nourish in the best way possible for the people we support, Kelis and her team also schedule regular family visits directly in the technology. This helps them plan daily activities while ensuring family commitments are recognised and prioritised. So, the people we support are empowered to maintain important relationships:
