Specialist Behavioural Support
Our Specialist Behavioural Support Services are designed to meet the unique needs of individuals with learning disabilities and / or autism, who have a range of additional needs such as mental health conditions, challenging behaviour, or forensic backgrounds.
We align support with the Transforming Care National Service Model, and we are committed to providing services that are flexible and preventative. Our goal is to support individuals with enhanced and complex support needs in their communities, preventing unnecessary hospital admissions and enabling successful transitions from secure or inpatient settings. We help individuals live as independently as possible within their communities so they can enjoy a good and meaningful life.
Early intervention and prevention
We believe that early intervention is key to preventing unnecessary hospital admissions. Through our “step-up” services, we offer intensive support to individuals at risk of being admitted to hospital, providing the right care at the right time.
For people preparing for discharge from hospital or secure settings, our “step-down” services focus on helping them transition smoothly back into the community. This includes developing skills for independent living, fostering community connections, and ensuring housing is adapted to meet specific needs while maintaining a homely feel.
Our approach
Our model of support centres on developing personalised care plans tailored to the individual. We work closely with the people we support, their families, and local professionals to co-produce plans that consider each person’s unique circumstances, needs, and goals.
