DownloadFlexible support
The support provided is always based on assessed need.
Interventions are typically short term, lasting from a few days up to four weeks, with regular reviews involving the individual, Commissioners and partner agencies.
Exit planning begins early, ensuring smooth transition back to community services and ongoing support where required.
Four-week support structure
The four-week support structure below provides an example of what support may look like. Specific timeframes and support options will vary depending on individual needs.
Week one – Rapid Assessment
Complete clinical screening to develop a co-produced Health & Wellbeing Care Plan and Staying Well Plan, to guide early intervention and support.
Week Two – Formulation
Gather important clinical data, analyse patterns, and develop a functional understanding of support to enhance current wellbeing and care planning.
Week Three – Coaching and guidance
Prioritise coaching and guidance around care plans, agreements on successful strategies, and finalise Nursing/Practitioner week four recommendations.
Week four and onwards – Follow-on Outcomes
Implement agreed strategies into practice, support handover where appropriate, and continue monitoring progress and relapse prevention.
Maintenance
Review long term support, strengthen skills and independence, update recommendations as needed, monitor wellbeing and continue regular reviews.
