With support at home you’ll receive high-quality, person-centred support care in the comfort of your own home. Delivered by specially trained teams, we’ll work with you to manage your needs and establish your independence.

Who can have support at home?

Support at home is an affordable alternative to hospitals and care homes. With the support of Healthcare Assistants, Support Workers, or Nurses, your care is delivered in familiar, comfortable surroundings.

Our person-centred approach

We use person-centred support, putting you at the heart of your care plan. We work with you to create a support plan tailored to meet your individual needs, values, and preferences.

As well as managing your health and wellbeing needs, we support you to set goals, embrace meaningful opportunities, and pursue your passions. As you reach your goals, we’ll work with you to set new ones, so you can continue living the fulfilling life you deserve.

We can support you with:

Care at home for children with complex care needs

We understand the importance of family and the responsibilities that come with supporting a child or young person with complex needs. With our specialist complex care support, we empower children and young people to receive support in the comfort of their own homes, surrounded by the people they love.

Our Children’s Complex Care team offers hospital-grade care and support to children and young people. Providing expertly trained Nurses and Healthcare Assistants, we ensure your child or young person is happy, healthy, and empowered to embrace the joys of childhood. Proven to reduce lengthy hospital stays, our complex care and support promotes independence and nurtures positive mental health and emotional wellbeing.

As well as providing essential clinical support, our dedicated teams extend our family values and person-centred ethos, supporting your child or young person to dream big and achieve their goals.
