Our transition approach
Our transition process has four key stages to ensure successful transition. Our priority is supporting you to transition with ease – whether you’re joining us for first time or moving along your support pathway, we aim to make the process as comfortable as possible, with minimal disruption to your usual routine.
Introductions
Your transition starts with an initial assessment. We’ll get to know you as an individual and build a better understanding of your needs and concerns, as well as your hopes, dreams, and ambitions for the future. We’ll also get to know the important people in your life, like your family and current support team. We’ll build a better understanding of your history, routine, and support, and start planning your transition!
Developing transition plans
We’ll arrange a transition planning meeting with you and your support network where we’ll co-create a detailed person-centred transition plan, so we can get to work and make your transition a meaningful and positive experience for you. We’ll map out steps we need to take, and the parts other people will play. Together, we’ll all support a smooth transition to your new home and tailor it to suit your needs.
Actioning the transition plan
When it’s time to start your transition, we’ll take it at a pace suited to your needs and wishes. Before you move in, we’ll embrace the fun stuff together. We’ll help you personalise your new home and identify any accessibility or sensory equipment you might need. We’ll help you to get sociable with your new housemates and decide on the team who will support you.
If you need some help with setting up bills and applying for benefits, we can help with that too! Once you’ve moved in, we’ll keep going with the transition plan and offer a unique “settling in” phase for the first three to six months in your new home.
Reviewing your transition plan
We’ll regularly review your support with you, your family, and other professionals involved in your care. In our review meetings, we’ll look at your progress since your transition, identifying what’s gone well and where we might need to do some more work.
We always want you to feel comfortable with your support. Our transition plans are designed to gradually reduce support as you feel more confident and independent in your new home. But you can be reassured that our support will remain flexible to your needs. We’ll keep you at the heart of decision making throughout your transition, ensuring you’re empowered to live the life you choose.
