Open day booking form

Our in-person open day will be guided by government advice for COVD-19. This will mean good ventilation, antibacterial stations, mask-wearing where possible, social distancing and COVID-19 testing before arriving at our service. To ensure that everybody is kept safe, we ask that you completed a lateral flow test that day and can evidence a negative result before entering our service. If you are selecting the ‘general drop-in’ option below, please ensure you phone our service 30 minutes in advance. This will ensure we can make sure social distancing can be maintained…and make sure the kettle is on for when you arrive!
