Jade’s transition from child services to adult services

Jade is a person we support at one of our supported living services. After receiving support at a children’s service from age 15 -18, it was time for her to transition to an adult service so she could gain more independence and confidence. That’s when Jade approached Voyage Care. We worked with her to make her transition as smooth as possible and ensured she received the support she needed to achieve her goals!

Jade’s journey to Voyage Care

Jade had received support for three years at a children’s service. She didn’t have any one-to-one time with her support workers and felt limited with what she could achieve. At her previous service, Jade didn’t have the opportunity to develop her social skills or make decisions on what she wanted to do in her spare time. But Jade had ambitions to become more active in the community and do daily tasks for herself – something she hoped would be possible when she moved to adult services.

Making the transition to adult services was a worrying time for Jade. Like any young adult leaving home for the first time, it was an important, life-changing event. Jade knew it would give her the opportunity to develop valuable living skills and allow her to take control of her future. She also knew it was important to choose a provider who would help her through the process, and beyond. Jade chose to transition to Voyage Care and we helped her every step of the way!

Preparing to transition

In preparation for the move, we had regular meetings with Jade and her support worker. We discussed how we could help her feel comfortable throughout the process and co-created a support plan with goals she wanted to achieve once she’d moved. We also ensured Jade met her new housemates so she could start building relationships with them before moving in.

Jade was involved in every step of the process, empowering her to make decisions about her care and support from the start. This helped her feel more confident about her transition to supported living. Our support didn’t stop there! We rolled up our sleeves to help Jade build flat-pack furniture, ready for her new bedroom. After a few weeks of our support, Jade’s excitement shone through! She was ready to move into her new home and start smashing her goals!

The road to success

When setting her goals, most Jade’s ambitions revolved around being more independent and improving her confidence. Our team built trusting relationships with Jade and her support team encouraged her to become more proactive in taking care of herself.

This involved small steps like making her bed and tidying her room every day, encouraging her to make healthy choices at mealtimes and supporting her to maintain a personal care routine. All these small steps empowered Jade to feel more confident. This new-found confidence helped Jade tackle her long-term goals, which included using public transport.

Jade wanted to take the bus to the local shops and do her own grocery shopping. When she was ready to work on this goal, Jade’s support team helped her plan her bus routes and encouraged her to keep in touch while she was out and about, so they knew she was safe. Jade now takes the bus every week by herself to do her grocery shopping!

Using public transport by herself was also important in Jade achieving her biggest goal – attending her local college to study catering and childcare. Our team knew how important this goal was to Jade and worked with her to make this happen. When she enrolled in college, our team liaised with the college SEND team (Specialist Education Needs and Disabilities) to ensure Jade would get the support she needed while studying there.

Enrolling at college empowered Jade to enhance her daily living skills like cooking – her speciality is chicken curry! Jade continues to evolve her culinary skills and has recently got into baking. She makes an incredible brownie and custard!

Jade’s transformation

Since transitioning to Voyage Care, Jade has made significant progress. Those closest to her have seen a real difference! Jade had limited opportunities to socialise and access the community before moving into to our supported living service. Now, she attends local day services twice a week, where she does arts and crafts, and develops her IT skills. She has also completed a course at a local animal therapy centre, where she learnt to feed and care for the animals! 

Jade’s support worker has assisted her throughout her transition to adult services and has also seen a huge transformation in Jade. They shared,

“Jade has come on leaps and bounds with developing her independent living skills and decision making. It’s an exciting time for Jade. She is creating some meaningful relationships. She should be so proud of herself and everything she is accomplishing!”
