Joan’s goal was to live independently again and return to fitness following a brain injury resulting from a cardiac arrest. With support from Voyage, the neuro-physiotherapist and Joan planned for her discharge from the outset.
Joan’s outcomes-focused programme of daily exercises and community leisure activities improved her mobility and overall fitness with the aim of reducing her dependence on others to complete everyday tasks.
Structure, use of consistent prompts and frequent practice improved Joan’s ability to complete tasks and communicate with confidence. This allowed a staged reduction in direct support such that she was able to return home requiring minimal support with activities of daily living.
