Referral Process

We have a strong reputation for our flexible and speedy response to referrals.

You are welcome to make an initial enquiry or seek advice and information using the contact details below. This website will give you an indication of current places available within our services, but we will also be aware of likely future vacancies which will come about as a consequence of planned movement, therefore an enquiry is always worthwhile.

After discussion we need to receive a formal request to undertake an assessment. After that we can move swiftly to make the necessary organisational arrangements. If there are tight time constraints we will do our very best to help.

The three key objectives of the assessment process are:

  • To assess whether we are able to meet the special needs of the person referred to us.

  • To determine if we have a suitable vacant place for that individual. In doing so we will seek to match both the skills and knowledge of the staff team and compatibility with existing service users.

  • To determine the initial care plan needs and appropriate level of support required, enabling us to produce a costed proposal of the required fees.

Before undertaking the assessment it is important that we have access to as much relevant information as possible relating to the current needs of each individual and it is also very helpful to see reports and records relating to their previous history.

We will determine who is most appropriate to take the lead in undertaking the assessment and agree with you what is the most appropriate time, place and method of assessing needs. It is most likely that the person taking the lead will be one of our referral and assessment managers. It is sometimes appropriate for more than one person to be involved in the assessment process or to undertake follow up visits.

We will produce a clear written report and if the assessment process suggest that we can meet the needs of the individual, and we have a suitable place available, we will send a fully costed proposal. Alternatively, if we are unable to meet the assessed needs or do not have a suitable vacancy we will also ensure that this is communicated to you at the earliest opportunity.
